Eagle Mountain Family Dental

We love to help as many people as we can, so we’re always accepting new patients of all ages to our dental office. If you’d like, you can shorten the duration of your first visit by completing our new patient forms right now! Of course if you’d prefer, you’re welcome to complete the new patient paperwork once you arrive. If you would prefer to complete the forms in our office, please arrive 15 minutes early to your appointment.

We deliver the finest care to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. If you have questions regarding your account, please contact us at (817) 444-3890. Many times, a simple telephone call will clear any misunderstandings.
Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
Dental Insurance
Your dental plan is a form of compensation your employer provides. You can expect the carrier (insurance company) to reimburse you for a portion of our fee. That portion is determined by the contract between your employer and the insurance company. The higher the premium paid by your company, the more generous the reimbursement.
Although we are not a party to the contractual arrangement with your insurance company, we do want to help you receive the maximum reimbursement to which you are entitled. As a convenience to you we will help you process your insurance claims in order for you to receive this maximum benefit. We will also gladly provide dental x-rays and a written diagnostic report should your insurance company have any questions about the services which were provided.
At all times, you can be confident that we will always provide you with our best services without regard to the limitations imposed by your insurance coverage. To do otherwise would violate our contract with you - the only contract we feel morally obliged to honor.
Billing Policies
We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. We can make arrangements for a monthly payment plan but this must be done prior to the actual procedure.

 First Dental Appointment
We gladly accept new patients to our growing family. We are a true family practice treating patients of all ages. Dr. Reeves bought the practice in 2000 from Dr. Rhone (who retired in 2000), which he had worked in for over thirty years. Currently we have some clients who have been coming to us for over twenty years, including some second generation families.
The practice philosophy is simple, help our valued patients reach optimum oral health in a way that meets their needs and expectations. There is often more than one way to solve a problem, and it is each individuals right to decide what treatment course they would like to follow. Ultimately, no matter what the patients needs and wants are, the most important thing is to start out right with a proper diagnosis. That is, through early detection and treatment anyone can have a healthy mouth in a cost effective matter.

With this philosophy in mind, all new patients get a thorough work up which includes...

  • Medical history review
  • Comprehensive history form followed by verbal interview. Many medical conditions complicate oral health and no treatment plan is complete without looking at the "big picture".
  • Dental history
  • Discussing past dental treatments helps define patient expectations, desires, and any anxieties, etc.
  • Soft tissue evaluation intra/extra oral
  • Soft tissues of the mouth, tongue, throat, lips, face, and neck are evaluated. Salivary glands are checked for function, enlargement, etc.
  • Oral cancer Screening
  • Done on every patient at every checkup
  • Periodontal Evaluation
  • Periodontal Screening and Recording done for new patients and at all check ups. PSR 3 or higher results in full periodontal exam
  • TMD Screening
  • Hard Tissue Exam
  • Teeth evaluated for decay, wear, signs of bruxism, erosion, fracture, etc. Occlusion (bite) evaluated.
  • Appropriate x-rays taken
  • Initial treatment plan proposed, more complex cases may require a second treatment planning appointment.

Insurance Information

We handle all claims for you. We submit all claims electronically, which speeds up turnaround time on claims. Predetermination of dental benefits are submitted for you as well, which aids in the budgeting and timing of your treatment

In addition to accepting all indemnity (traditional) insurances and payments from non participating PPOs, we participate in the following plan.

  • Cigna

Please assist us by providing the following information prior to the time of your first visit:

  • A list of medications you are presently taking
  • If you have dental insurance, bring the necessary information. This will allow us to process your claims for you.
  • IMPORTANT: All patients under 18 must be accompanied by a parent or guardian at the initial visit.

If your previous dentist has taken x-rays within the past year, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. We will be happy to make copies and return the originals to that doctor. If additional films are necessary, they can be taken at our facility.
